The Joint Desk "Clinical documentation copy" is available to provide:
- copies of medical records, outpatient or emergency room reports, X-rays (X-rays, CT, MRI);
- information on the characteristics of prostheses, plates, screws, etc. implanted for the execution of magneting resonance imaging.
The desk is located at the main entrance of the Rizzoli Hospital (via G. C. Pupilli 1, 40136 Bologna) and is open to the public on Tuesdays and Fridays from 11.00 to 13.00. For general information and delivery times contact the telephone number +39051-6366343 active on Mondays, Wednesdays and Thursdays from 9.00 to 11.00.
Request procedure for clinical documentation
Users assisted in Emilia-Romagna, for hospitalizations with a discharge date after 10 March 2025, will have the opportunity to download the simple copy of their medical record free of charge by accessing their Electronic Health Record (ESF) via SPID, CIE and CNS without the need to request it. In other cases the request must be transmitted exclusively using one of the following methods:
- Those who have a digital identity (SPID/Electronic Identity Card and National Service Card) will be able to book and download their medical records independently through this platform.
- by contacting the help desk in person or by delegating a person other than the person concerned, with a written proxy, a copy of the patient and delegate identity document;
- leaving the request in the appropriate box at the counter, when the counter is closed, with a copy of the patient's identity card;
- by sending the form via PEC/e-mail, in pdf format, to the address ufficio.copie@pec.ior.it or ufficio.copie@ior.it;
- by letter addressed to: Clinical Documentation Archive, Rizzoli Orthopedic Institute, Via G. C. Pupilli n.1, 40136 Bologna.
Those who request a copy of the file from the Health Documentation Office of the Rizzoli Orthopedic Institute must specify whether it is a plain copy or a certified copy.

It is still possible to request a copy from the Health Documentation Office of the Rizzoli Orthopedic Institute, specifying whether a simple copy or a certified copy is required and the date of the service.
Method of requesting information on implanted materials
Requests for information on the characteristics of prostheses, plates, screws, etc. should be sent one month in advance by contacting the Health Documentation Office in the same way.
Information necessary to request health documentation
- personal data: surname and first name, place and date of birth;
- telephone number (mobile number essential for requests to be sent via the web), address where the copy can be delivered;
- sign the request and attach a photocopy of a valid identity document;
- PEC/mail address;
- if the request relates to a minor, the parent must self-certify parental responsibility (here is the relevant pdf form or doc form);
in the case of a deceased person it is necessary to complete a self-certification of the status of family member or heir (here is the relevant pdf form or doc form); - in the case of a guardian it is necessary to complete a self-certification of the status of guardian with details of the provision (here is the relevant pdf form or doc form);
- in the case of a support administrator it is necessary to complete a self-certification of the status of support administrator with details of the provision (here is the relevant pdf form or doc form).
Withdrawal method
The collection of medical records, outpatient reports, diagnostic reports, emergency room reports can take place according to one of the following methods:
- via Electronic Health Record (EHR)
- through the Ready Folder platform for copying medical records;
- via the web (service available only for medical records with hospitalization from 1975 to the present);
- requesting shipping by post with costs borne by the recipient;
- by contacting the counter directly. In this case, it is recommended to contact the Health Documentation Office to verify that the copy is ready.
The collection of information on implanted materials can only take place via PEC, collecting it at the counter or by post.
The clinical documentation requested from the Health Documentation Office of the Rizzoli Orthopedic Institute may be withdrawn exclusively by:
- person (required to show valid identity document);
- delegated person in writing (with the obligation to show your valid identity document and copy of the patient's document) (pdf form or doc form).
Attention! In the case of a minor, deceased person, person subject to guardianship/curatorship/support administration, the self-certifications and measures indicated for submitting the request are necessary (here is the relevant pdf form or doc form).
X-rays
For x-rays taken up to 31/1/2007: It is possible to request viewing of the x-rays taken at our hospital either on an outpatient basis (Polyclinic or Emergency Department) or during hospitalization.
For X-rays taken since 1/2/2007: Plates taken during an hospitalization or during an outpatient service (Emergency Room or Specialist Outpatient) can be requested from the Health Documentation Office in a copy on CD.
For X-rays taken since 1/5/2017: X-rays taken during a hospitalization or during an outpatient service (Emergency Room or Specialist Outpatient) can be requested through the ESF for citizens of Emilia Romagna and through the online report portal for citizens assisted outside the region.
Copies on CD and x-rays can be collected by showing up directly at the counter or received by post with shipping costs paid by the recipient.

Payment procedure
Payment for the health documentation requested in copy or collected at Counter 1 must be paid at the time of the request using the automatic cash register present in the Institute. Alternatively, payment can be made via PagoPA channels (e.g. tobacconists, post office, ATM, Pagonline ...) following the instructions on the payment forms.
The user who requests the sending of the documentation to their home, via the web or collection at the counter via e-mail/PEC will receive the request reminder and the form to make the payment by post/e-mail/PEC.
There is no payment for the issue of the FIRST COPY of the MEDICAL RECORD, without prejudice to the costs incurred for any shipment. For folder copy requests after the first and for the remaining documentation the costs are as shown below:
